1. Investment Highlights

Eli Lilly (LLY.US) is scheduled to report its Q3 2026 results before the U.S. market open on October 29, 2026, with the conference call set for 10:00 a.m. ET. Wall Street consensus expects total revenue of approximately $22.6 billion (about +28.0% YoY, versus $17.6 billion in the year-ago period), with adjusted EPS largely clustered in the $5.7–$6.3 range (down from $7.02 a year earlier, partly due to potential acquisition-related R&D charges). The market’s main focus is whether the two blockbuster drugs, Mounjaro and Zepbound, can continue their rapid growth, and whether the newly launched oral weight-loss drug Foundayo can truly ramp up.

2. Four Key Focus Areas

Focus 1: Can Mounjaro and Zepbound Continue to Beat Expectations

The tirzepatide franchise remains the core growth engine. In Q2, the two products together generated about $14.9 billion (nearly 65% of total revenue), with Mounjaro at approximately $9.94 billion (+91.0% YoY) and Zepbound at about $4.93 billion (+46.0% YoY). For this quarter, the market expects Mounjaro at roughly $10.0–10.1 billion and Zepbound at about $5.0–5.1 billion. Key items to watch include whether U.S. volumes are affected by cash-pay price cuts and the Medicare Bridge program, and whether international growth (especially in China) can offset pricing pressure.

Focus 2: Ramp-Up Pace of the Oral GLP-1 Foundayo

Foundayo (orforglipron), a once-daily oral weight-loss drug approved in April, generated only $98 million in Q2. This quarter marks the first full period after its inclusion in the Medicare GLP-1 Bridge (eligible patients can access it for a $50 monthly copay starting July 1). The market expects revenue to rise to approximately $230 million. Investors will focus on new patient starts, dose titration speed, and competition with Novo Nordisk’s oral Wegovy.

Focus 3: Room to Raise Full-Year Guidance and the “Beat and Raise” Tradition

The company currently guides full-year revenue of $85–87 billion and non-GAAP EPS of $35.50–$36.50. First-half revenue has already reached $42.77 billion. If this quarter significantly exceeds consensus, the market expects full-year guidance to be raised toward $88 billion or higher. Lilly has beaten expectations and raised guidance in recent quarters; continuing this pattern would further reinforce the “obesity drug super-cycle” narrative.

Focus 4: Pricing Pressure, Capacity, and Gross Margin Resilience

Despite strong volume growth, realized prices remain under pressure (Q2 saw roughly 60% volume growth with about a 13% price decline). This quarter’s results will show whether the price decline widens into the mid-teens and whether gross margins can hold at high levels. Progress on capacity expansion and supply relief will also influence the growth trajectory for the second half and next year.

3. Risks and Opportunities

Upside Catalysts:

  • Combined Mounjaro and Zepbound growth significantly exceeds expectations, with accelerating international volume.
  • Foundayo revenue clearly surpasses $230 million, with improving new patient start share.
  • Full-year revenue guidance is raised above $88 billion, accompanied by positive second-half commentary.

Downside Risks:

  • Wider price declines or sequential slowing in U.S. volumes, raising questions about growth sustainability.
  • Foundayo ramp-up falls short of expectations, or oral competition intensifies.
  • Results merely meet guidance without a raise, compounded by potential R&D charges weighing on EPS.

4. Trading Strategy

Bull Case: If revenue clearly beats consensus, the GLP-1 dual engines and Foundayo both demonstrate strength, and management raises full-year guidance, it could reinforce Lilly’s position as the undisputed leader in obesity drugs and support further valuation expansion and new highs.

Bear Case: If core product growth slows, pricing pressure exceeds expectations, or the guidance raise is insufficient, it could trigger profit-taking, especially with options pricing in roughly a 5% move.

Key Data to Watch:

  • Total revenue and year-over-year growth
  • Mounjaro and Zepbound sales along with volume/price breakdown
  • Foundayo revenue and new patient trends
  • Whether full-year revenue and EPS guidance is raised

Operational Suggestion: If results beat expectations with positive guidance, consider opportunities on any post-earnings or next-day pullback. If the data is flat or guidance is cautious, watch for short-term volatility and avoid chasing. Position sizing and stop-losses should be adjusted according to individual risk tolerance.
